Medical and administrative forms are a routine part of every US healthcare encounter. New patients typically complete a set of forms before their first appointment, and some forms — such as consent to treat and HIPAA authorizations — must be completed at each facility. This forms library will provide translated reference versions of the most common form types to help patients understand what they are being asked to provide before they sign.

These are reference translations intended to help patients understand the content and purpose of standard US medical forms. They are not intended to replace official forms provided by a specific facility, which may differ in content and must be completed as-issued by the provider. Forms will be published in Batch U4.

Languages covered

All form translations will be available in English, Chinese (Simplified), Korean, Portuguese, Spanish, Filipino (Tagalog), Thai, Vietnamese, and Japanese — the nine languages covered across the AMDA-IMIC US section.

Language access is a legal right at any healthcare facility receiving federal funding. Under Section 1557 of the Affordable Care Act, you are entitled to a qualified medical interpreter at no cost.
