This site is a static reference archive. It does not run user accounts, store personal data in a database, or process payment information. The information practices described here reflect the limited technical infrastructure of a read-only reference site.

Hosting and Infrastructure

HTTP access logs at the infrastructure level may include: IP addresses (typically anonymized), request paths, timestamps, HTTP status codes, and browser user agent strings. Analytics

This site may use aggregate analytics to understand which pages are visited and where readers come from by region. Any analytics in use are configured to collect only aggregate, anonymized data — page views and referrer domains, not individual user identifiers or session recordings.

No behavioral tracking profiles are built from visits to this site. No data collected from this site is sold or shared with advertising networks.

Forms Library

The forms documented in the forms library are described for reference purposes only. This site does not host fillable forms, does not accept form submissions, and does not collect personal health information. If you require an actual administrative form for use with a Japanese medical institution, you must obtain it from the relevant institution directly.

Children

This site is a general health reference resource. It does not knowingly collect personal information from children under the age of 13. The site does not operate any features that require account registration or submission of personal information.
