About This Collection

Japan requires substantial administrative documentation in healthcare and occupational health contexts. Foreign residents face the challenge of completing these forms in Japanese, a language most do not read fluently. AMDA-IMIC's forms library addressed this by providing translated reference versions of each form type, showing the purpose of each field and how to complete it correctly.

The 12 form categories in this library cover the most frequently needed document types: patient registration, consultation appointments, health examinations, insurance-related claims, and occupational health compliance documents required under Japan's Industrial Safety and Health Act.
