Health Insurance Premium
保険料 (hokenryō)
The health insurance premium (保険料, hokenryō) is the monthly payment required to maintain health insurance coverage in Japan. For National Health Insurance, premiums are calculated by each municipality based on prior-year income, household size, and fixed per-person charges. For Shakai Hoken, premiums are a percentage of monthly salary split between employer and employee. Persistent non-payment can result in benefit suspension or legal enforcement action.
NHI premiums vary considerably between municipalities; low-income residents or those with no income may apply for a reduction or full exemption at their local municipal office.
