Insurance Claim
保険請求 (Hoken seikyū)
A formal request submitted to an insurance provider for reimbursement or direct payment of covered medical expenses. In Japan, public health insurance claims are processed automatically between the clinic and the insurer; however, patients must file separately for high-cost medical expense rebates, overseas treatment reimbursement, or benefits from supplemental private policies. Claims typically require receipts, itemized bills, and a physician's certificate.
For the high-cost medical expense (kōgaku ryōyōhi) rebate, file with your insurer or municipality within two years of the treatment month.
