診療明細書 (Shinryō meisaisho)

A detailed breakdown of all medical services, procedures, medications, and fees charged during a hospital visit, separate from the standard receipt. Since 2010, Japanese hospitals have been legally required to provide an itemized bill upon request at no charge. This document is valuable for verifying billing accuracy, understanding what was treated, and supporting private insurance reimbursement claims.

Request the meisaisho at discharge or the billing window; unlike the receipt, it lists individual diagnostic codes and procedure points used to calculate the fee.
