Advance Payment Exemption

限度額適用認定証 (Gendo-gaku tekiyō nintei-shō)

A certificate obtained in advance from the health insurer that caps the amount a patient pays at the hospital counter for a single month, preventing the need to pay large sums upfront and then seek reimbursement. Without this certificate, patients must pay the full copayment amount and later file for a high-cost medical expense rebate. It is particularly important for planned surgeries or hospital admissions.

Apply for the nintei-shō at your insurer's office or online before hospital admission; income bracket determines the monthly cap amount.
