What it means

Value-based care is a healthcare payment and delivery model that rewards providers for achieving better patient health outcomes rather than simply for the volume of services they deliver. Under value-based contracts, providers receive bonuses for hitting quality and efficiency targets — such as reducing hospital readmissions or controlling chronic disease — and may face penalties for poor outcomes. For patients, value-based care is intended to mean better coordinated, more patient-centered treatment. Common value-based arrangements include Accountable Care Organizations (ACOs), Patient-Centered Medical Homes (PCMHs), and bundled payment programs. Medicare and private insurers increasingly use these models.
