What it means

A formulary tier is your health insurance plan's way of grouping covered drugs by how much you pay out of pocket. Most plans have three to five tiers, with Tier 1 covering the least expensive preferred generic drugs and higher tiers covering brand-name or specialty drugs that cost more. The tier determines your copay or coinsurance for each fill. If your medication falls on a high tier, you can ask your doctor about lower-tier alternatives or request a formulary exception from your insurance plan if no alternative is appropriate. Checking the formulary before starting a new prescription can prevent unexpected costs at the pharmacy.

How to say it

Sentences a patient might use with a doctor, nurse, pharmacist, or insurer:

What tier is this medication on my insurance formulary?

¿En qué nivel está este medicamento en el formulario de mi seguro?

My medication moved to a higher tier this year, so my copay went up.

Mi medicamento pasó a un nivel más alto este año, por lo que mi copago aumentó.

Is there a Tier 1 generic that works the same way as this brand-name drug?

¿Hay un genérico de nivel 1 que funcione igual que este medicamento de marca?
