Medications
An accurate medication list is essential for safe care in the United States. At every clinical encounter — whether a primary care visit, specialist appointment, urgent care visit, or emergency room presentation — providers will perform medication reconciliation, a process of reviewing and confirming every medication you currently take. This includes prescription drugs, over-the-counter (OTC) medications such as ibuprofen or antacids, vitamins, herbal supplements, and even topical products like medicated creams or patches. Many patients underestimate how important supplements and OTC medications are to disclose. In the US, herbal supplements are not regulated as drugs, which means interactions between supplements and prescription medications are common and sometimes dangerous. For example, St. John's Wort can reduce the effectiveness of birth control pills and certain antiretrovirals. Fish oil and vitamin E in high doses can increase bleeding risk. Providers cannot manage these risks if they do not know about them. In the United States, medications are prescribed with specific instructions about frequency — once daily, twice daily (BID), three times daily (TID), or as needed (PRN). These instructions also often specify conditions, such as "take with food," "take on an empty stomach," or "take at bedtime." Following these instructions correctly affects how well the medication works and minimizes side effects. If you are pregnant or could become pregnant, it is critical to tell your provider before any new medication is prescribed. Many medications carry FDA pregnancy categories that guide prescribing decisions, and the provider needs this information to select the safest option for you.

Key terms
- generic drug
- A medication that contains the same active ingredient, in the same dose and form, as a brand-name drug whose patent has expired. Generic drugs must meet the same FDA standards for safety and efficacy as their brand-name equivalents and are typically much less expensive.
- brand name
- The proprietary name given to a drug by the manufacturer that first developed it. For example, Lipitor is the brand name for atorvastatin. Brand-name drugs are usually more expensive than their generic equivalents.
- dosage
- The specific amount of a medication to be taken at one time, along with how often it should be taken. A complete dosage instruction includes the dose (e.g., 500 mg), the frequency (e.g., twice daily), and sometimes conditions (e.g., with food).
