Describing Pain and Symptoms
Accurately describing pain and symptoms is one of the most critical communication tasks in any medical encounter. In the United States, clinicians are trained to assess pain using a standardized 0-to-10 numeric scale, where 0 means no pain and 10 means the worst pain you can imagine. This scale is used across emergency rooms, urgent care centers, inpatient floors, and outpatient clinics, so being familiar with it helps staff calibrate their response and document your condition accurately. Beyond a number, providers need to understand the quality of your pain — whether it is sharp (like a knife), dull (like a deep ache), burning, throbbing (pulsating in rhythm with your heartbeat), or pressure-like (as if something is pressing on you). They also need to know where the pain started, whether it has moved or radiated to another part of the body, how long you have had it, and what makes it better or worse. Describing aggravating factors (activity, eating, lying down) and relieving factors (rest, ice, over-the-counter medication) saves significant time. In the US emergency department, a nurse will conduct triage — the process of sorting patients by urgency — before you see a physician. Vital signs (blood pressure, heart rate, temperature, respiratory rate, oxygen saturation) are taken during triage. If you are experiencing chest pain, difficulty breathing, or symptoms of a stroke (sudden numbness, confusion, trouble speaking, or severe headache), tell the triage nurse immediately — these are high-priority emergencies and you may be triaged to the front of the line. Never minimize symptoms when speaking to clinical staff; the United States healthcare system is designed to escalate quickly when warranted.

Key terms
- vital signs
- The four basic measurements that indicate a patient's body status: blood pressure, heart rate (pulse), body temperature, and respiratory rate. Oxygen saturation (pulse oximetry) is often considered a fifth vital sign in US clinical settings.
- triage
- The process of sorting patients in an emergency department based on the urgency of their condition. A triage nurse assigns a priority level so that the most critically ill patients are seen first regardless of arrival order.
- symptom
- A subjective experience reported by a patient, such as pain, nausea, or dizziness, as distinguished from a sign, which is an objective finding measurable by a clinician, such as elevated blood pressure or fever.
