Retatrutide Regulatory Status and Research Supply

Retatrutide is unapproved and cannot be compounded. Its phase 3 readouts, filing timeline, expanded access route and what research-use supply means.

Retatrutide is an investigational drug with no marketing approval in any country, and in the United States it cannot lawfully be compounded. As of September 2026 its phase 3 program has reported in obesity, obesity with type 2 diabetes, obesity with cardiovascular disease and knee osteoarthritis, and its sponsor has said it plans to file with FDA in the first quarter of 2027. Until an approval exists, there are three ways the compound is supplied: clinical trials, a sponsor-run expanded access program, and laboratory research material, which is not a medicine and is not for human use.

Neither. FDA’s position is direct: retatrutide cannot be used in compounding under federal law, it is not a component of any FDA-approved drug, and it has not been found safe and effective for any condition (FDA, concerns with unapproved GLP-1 drugs). The same page records warnings to telehealth companies marketing retatrutide to consumers, to ingredient distributors selling it to compounders, and to companies selling it “falsely labeled ‘for research purposes’ or ‘not for human consumption’” while supplying it for human use with dosing instructions.

That last point defines what research supply is and is not. A research-use-only designation describes a reagent sold for laboratory work to qualified organizations. It does not create a pathway for personal use, and FDA’s enforcement record shows the label offers no protection when the surrounding marketing points at people. What does the clinical record show so far?

Two phase 2 trials established the dose range. In obesity, 338 adults received once-weekly retatrutide at 1 mg to 12 mg or placebo for 48 weeks; the 12 mg arm reached a least-squares mean weight change of −24.2% against −2.1% on placebo (Jastreboff et al., N Engl J Med 2023). In type 2 diabetes, 281 adults were randomized against placebo and dulaglutide 1.5 mg, and HbA1c fell by 2.02 percentage points at 12 mg against 0.01 on placebo at 24 weeks (Rosenstock et al., Lancet 2023). A body-composition substudy of that trial found total fat mass fell 26.1% at the pooled 8 mg dose against 4.5% on placebo at 36 weeks, though only 103 of 189 enrolled participants had both scans (Coskun et al., Lancet Diabetes Endocrinol 2025).

The registrational program was designed as four phase 3 studies enrolling over 5,800 participants, with primary endpoints of percent weight change, Apnea-Hypopnea Index change for sleep apnea and WOMAC pain change for knee osteoarthritis (Giblin et al., Diabetes Obes Metab 2026). The head-to-head study against tirzepatide is the first direct comparison with an approved incretin; until it reads out, differences between retatrutide and the approved agonists come from separate trials. When could retatrutide be approved?

Not before 2027. The sponsor has said it will submit an application to FDA in the first quarter of 2027, later than an earlier end-of-2026 expectation, because it needed more time to compile manufacturing and quality-control data (as reported by BioPharma Dive, July 2026). Standard review after submission would put any decision later still.

A regulatory question sits under that filing. Under the Public Health Service Act a protein is defined as an alpha amino acid polymer of more than 40 amino acids, and products meeting that definition are licensed as biologics with 12 years of reference-product exclusivity, against five years of new chemical entity exclusivity for a drug. Retatrutide contains 41 residues, but FDA’s position, as reported in August 2026, is that it does not contain 40 alpha amino acids and should be regulated as a drug; the sponsor argues it qualifies as a biologic. The outcome decides how long the originator product would be protected from lower-cost versions, which matters for the eventual price discussed in what determines retatrutide cost. The same line has moved before: synthetic peptides above 40 residues that had been approved as drugs, including tesamorelin, were transitioned to biologics licenses on March 23, 2020.

Can anyone receive retatrutide outside a trial?

Only through the sponsor’s expanded access program, which is a physician-initiated, individual-patient route rather than a supply channel. The registry record lists eligibility as adults with a BMI of at least 35 kg/m² despite current weight-management therapy, two or more serious obesity-related complications, and no access to a comparable trial, with standard options including bariatric surgery discussed first (NCT07629401). Requests come from the treating physician to the sponsor, and availability depends on eligibility, geography and supply.

Everything else is trial enrollment. The phase 3 records in Table 1 list sites and eligibility criteria, and new studies, including a phase 3 in obesity with chronic low back pain and a weight-maintenance study, appear on the same registry.

What does research-use supply mean for a laboratory?

It means a reagent, documented by its certificate of analysis, supplied to qualified organizations for in-vitro and animal work. The obligations are the purchaser’s: hold the material under institutional handling rules, record the lot number against the work it supports, and keep the certificate with the experimental record, because that document is what ties a result to a specific batch.

Three analytical fields decide whether a labeled milligram figure describes the solid in the vial. A certificate should carry the vial’s own batch number, report HPLC purity as area percent, and confirm identity by mass spectrometry against the sequence’s theoretical mass; purity alone says one species dominates, not that it is retatrutide. HEEZ Research is one supplier that states each lot carries a batch-specific third-party certificate covering HPLC purity and MS identity. Net peptide content is the fourth field and the most often missing: lyophilized material is peptide plus counterion, water and residual solvent, so without it the labeled mass is an upper bound on the peptide present.

Handling on arrival completes the record. A lyophilized cake should be dry, intact and uniform; a collapsed or glassy residue indicates a temperature excursion in transit. Concentration after reconstitution is labeled mass times net peptide content divided by diluent volume, so a vial labeled 10 mg at 90% net peptide in 2 mL of diluent holds 4.5 mg/mL of peptide, not 5. Dry powder keeps far longer than solution, which argues for reconstituting one vial at a time.

Why do some listings call retatrutide GLP-3?

It is trade shorthand with no basis in the literature. There is no GLP-3 receptor; retatrutide is described in its trials as a single peptide acting at the GIP, GLP-1 and glucagon receptors (Rosenstock et al., Lancet 2023), and its development code is LY3437943. A listing that uses the shorthand should still state the sequence and molecular weight, which are what an identity test is checked against.

Common questions about retatrutide’s status

Is retatrutide FDA approved? No. It is investigational, with an FDA submission planned for the first quarter of 2027.

Can a compounding pharmacy make retatrutide? No. FDA states it cannot be used in compounding under federal law.

Does a research-use-only label make retatrutide legal to use personally? No. FDA has warned sellers whose research-labeled products were sold for human use; the label does not change what the product is.
