How is prescription tesamorelin different from research-grade tesamorelin?

The molecule is the same. Almost everything around it is different.

The prescribing information describes tesamorelin as a synthetic analog of human growth hormone-releasing factor: the full 44 amino acid sequence, with a hexenoyl group, a six-carbon chain carrying a double bond at position 3, attached to the tyrosine at the N-terminus. It is prepared as an acetate salt. A research vial that is what it claims to be contains that same peptide.

What differs is the finished product. The approved drug is a formulated, sterile, preservative-free lyophilized powder. The current formulation lists 11.6 mg of tesamorelin per vial, equivalent to approximately 11.9 mg of tesamorelin acetate, together with hydroxypropyl betadex and mannitol as inactive ingredients. The label also states that the formulations are not substitutable with one another, because strength, reconstitution and storage differ between them. It matters more here than for short peptides, because a 44-residue chain offers many more positions at which a synthesis can drop or truncate a residue, and those byproducts are close to the target in structure.

Purity by HPLC. High-performance liquid chromatography reports area percent, the share of detected material that is one dominant species. It says nothing about which species that is. A listing that shows purity without identity, or identity without purity, has reported half of what you need.

Two details are specific to this molecule. The single sulfur atom in that formula belongs to a methionine residue, and methionine is a common site of oxidation in peptides. An oxidized form weighs about 16 Da more than the parent, so it is visible to mass spectrometry, and its presence in a lot is a storage and handling signal worth looking for. Second, the approved product’s own label reports vial content two ways, as tesamorelin and as tesamorelin acetate, and the two masses differ. That is the distinction net peptide content captures on a research certificate: labeled milligrams are solid mass, and peptide is only part of the solid. When net peptide content is missing, treat the labeled figure as an upper bound.

In the 26-week randomized trial published in the New England Journal of Medicine, 412 patients with HIV and abdominal fat accumulation, 86% of them men, received a daily subcutaneous injection of 2 mg of tesamorelin or placebo (PMID 18057338). The primary endpoint was percent change in visceral adipose tissue on computed tomography. Visceral fat decreased by 15.2% on tesamorelin and increased by 5.0% on placebo. Triglycerides fell by 50 mg per deciliter against a rise of 9 mg per deciliter, and IGF-I rose by 81.0% against a fall of 5.0%, with P<0.001 for each comparison. Adverse events did not differ significantly between groups overall, though more patients in the tesamorelin group withdrew because of one. Glycemic measures did not differ significantly.

Two qualifications belong next to those numbers. The effect depends on continued exposure: visceral fat reductions held to week 52 in the extension phases of the two 26-week trials, while discontinuation was followed by reaccumulation (PMID 22050344). And the population was specific. These were adults with HIV-associated abdominal fat accumulation, not a general population, and the approval follows the trial.

Whether the effect extends further is an open research question rather than a settled one. TRIUMPH, a 48-week trial registered as NCT06554717, is testing 24 weeks of tesamorelin or placebo alongside a home-based exercise programme in 100 adults with HIV, followed by a 24-week extension (PMID 42419889). It has published a protocol and no results.
